The immediate, technical causes of an accident are investigated. However there are underlying causes concerned with ways of avoiding the hazard and with weaknesses in the management system which are often not identified.The aim of this book is to show, by analyzing accidents that have occurred, mainly, but not entirely in the chemical industry, how we can learn more from accidents. The incidents discussed range from the trivial to major accidents such as Flixborough, Seveso and Bhopal. The author was formerly at KI, UK.
