35% of deaths in the US were attributed to heart disease in 1968, compared with only 8% in 1900. This epidemic trend has been reversed over the past two decades, however, with a decline of about 40% in the rate of heart disease mortality. Primary prevention and better clinical care have both contributed to this, but they are not the full explanation. Based on a conference held at the National Heart, Lung, and Blood Institute in December 1986, this book examines the reasons for the decline. It reviews trends and variation in morbidity and mortality from coronary heart disease, and in the use of clinical procedures to diagnose, treat, and prevent it. With contributions from both cardiologists and epidemiologists, it assesses the extent to which medical care has influenced the prevalence, incidence, mortality, and case fatality rates for coronary heart disease. The effects of such procedures as antihypertensive drugs, intravenous thrombolysis, cardiac pacemakers, coronary angioplasty, coronary bypass surgery, cholesterol-lowering agents, and cardiopulmonary resuscitation are examined, forming the most complete attempt available at answering this important medical question.
