This two-volume work aims to respond to the needs of orthopaedists, surgeons, neurosurgeons and rehabilitation specialists for a comprehensive clinical reference on the overall management of adults with acquired and congenital diseases and disorders of the spine. More than 100 contributors provide an account of the epidemiology, clinical presentation, diagnosis, prognosis, surgical and nonsurgical management of rheumatologic, metabolic, endocrinologic, neuromuscular, degenerative, neoplastic, infectious and trauma related disorders of the cervical, thoracic, thoracolumbar and lumbar spine. The first volume begins with discussions of social and economic issues that influence the treatment and the quality of life of patients with spinal disorders. Among the topics addressed are work incapacity, clinical evaluation and impairment ratings, functional restoration, and the role of pain clinics. The contributors then establish a systematic approach to diagnosis using laboratory, electrodiagnostic and imaging studies and examine the relative merits of conventional radiography, myelography, computed tomography, discography, spinal angiography, and magnetic resonance imaging in different clinical situations. A section on generalized disorders of the spine encompasses metabolic disorders such as osteoporosis, osteomalacia, and Paget's disease; vascular anomalies of the spinal cord; degenerative conditions; inflammatory disorders, including rheumatic disorders and infectious diseases; primary benign and malignant neoplasms of the spine; and metastatic tumours of the spine. The second volume provides coverage of disorders specific to each anatomic region of the spine - cervical spine, the thoracic and thoracolumbar spine, the lumbar spine, and the sacrum and coccyx. Illustrated analyses of virtually all available surgical options, including laminectomy, microsurgical laminotomy, laminoplasty, lumbar decompresion, spinal fusion, percutaneous and suction discectomy, and microdisectomy, as well as the implantation of prosthetic intervertebral discs and internal fixation devices are provided. Possible intraoperative and postoperative complications and long-term sequelae are identified, and the expected stages of recovery and rehabilitation are characterized. Attention is also given to nonsurgical treatments such as physical therapy, manipulation, chemonucleolysis, orthoses, and bracing, as well as to rehabilitation programmes.
