This new edition provides 75 nursing care plans for the care of adult clients with common medical-surgical conditions. Each plan, based on the nursing process, includes all relevant nursing and collaborative diagnoses, in-depth aetiologies, measurable outcome criteria, nursing interventions and selected rationales. Information on physical assessment diagnostic tests and pharmacological therapy is integrated into each plan. It contains more detailed information on individualizing care plans to meet each client's needs; a new index to make it easier to find specific nursing diagnosis; a new appendix listing all nursing diagnoses approved by NANDA, complete with definitions, related factors and defining characteristics (may eliminate a need for a separate nursing diagnosis handbook). It focuses on discharge planning and client teaching. An overview of each medical or surgical condition provides appropriate pathophysiological background. Very specific aetiology statements reveal the phsyiological or psychological basis of each nursing diagnosis. In-depth selected rationales enhance an understanding of the relationships of nursing interventions to nursing diagnosis and aetiology.
