This book serves to bridge the gap between intensive neurophysiological, pharmacological and psychiatric studies in the field of epilepsy and the everyday practical care problems in its diagnosis, the many aspects of drug treatment, the many new antiepileptic drugs under development and, the management of comprehensive care programmes. The emphasis is on application to patient management and this book represents the first European contributed volume on the state-of-the-art of practical care of the patient with epilepsy. Advances in techniques for intensive neurodiagnostic monitoring have played an increasing role in the understanding of individual seizure types and syndromes of the epilepsies. These are described together with recently standardized concepts and terminologies. Also included is the international consensus on the classification of the epilepsies. Diagnostic advances, including neuroimaging techniques, have also added to our understanding of the aetiology of seizures and these, together with the basic scientific advances resulting from them, are presented. As a consequence of our increased understanding of the biochemical and physiological substrates of epilepsy, progress has been made in the development of specific agents aimed at the modification of biochemical brain mechanisms. Therefore to complete the volume, new drugs, and those which are currently available, are reviewed with regard to when and how to begin drug therapy, different treatments for different groups of patients and, finally, how to shift or stop treatment.
