This timely issue reviews new therapeutic strategies for preventing or delaying renal disease progression. Special attention is given to pharmacologic interventions aimed at inhibiting the synthesis of angiotensin II or blocking its biological activity at the receptor level. The papers presented in this issue provide thorough coverage of the topic from the role of the renin-angiotensin system in the local control of renal function to the protective effects of ACE inhibitors in diabetes. Taken together, the studies document that, when urinary protein excretion is reduced by pharmacologic interventions, a proportional reduction in the rate of renal disease progression can be achieved. Clinical trials are now being carried out to investigate whether angiotensin II receptor blockade offers advantages compared to ACE inhibitors in slowing renal disease progression.
