Design, manufacturing, maintenance, and operating professionals often do not have the opportunity for meaningful dialoge. Even when a complete failure analysis is performed, insights gained about how to improve a process or material specification is often not relayed back to the designers. Many failures could be prevented if those responsible for making critical decisions had more information, especially regarding previous problems.
