Design, manufacturing, maintenance, and operating professionals often do not have the opportunity for meaningful dialoge. Even when a complete failure analysis is performed, insights gained about how to improve a process or material specification is often not relayed back to the designers. Many failures could be prevented if those responsible for making critical decisions had more information, especially regarding previous problems. This May 2000 conference brought together product designers and materials engineers to share knowledge gained over the last 20 years in fractography, stress analysis, and interdisciplinary approaches to engineering in general and failure analysis in particular.
